How Grant Payments Are Made

When you’ve been informed your grant will be funded by the Foundation, you may spend the funds authorized by the grant. There are two ways to make payments with grant funds.

  1. Purchase the materials or services specified in the grant and get a receipt for your payment. Print out a Check Request Form, attach your receipt(s), and sign the form. Click here to download the form. Please make a copy of the completed form for your records. Place the form in the Foundation mail box in the Main Office at South or mail the form and receipts to South High Foundation, 3131 19th Ave S, Minneapolis, MN 55407. The Foundation will prepare a check made out to you to reimburse you for the expenses. It take 10-14 days after we receive the Check Request From and receipts to process the payment and mail you a check.
  2. Have the vendor providing the materials or services send you an itemized invoice. Print out a Check Request Form, attach the invoice, and sign the form. Click here to download the form. Place the form in the Foundation mail box in the Main Office at South or mail the form and receipts to South High Foundation, 3131 19th Ave S, Minneapolis, MN 55407. The Foundation will prepare a check made out to the vendor to pay the invoice. It take 10-14 days after we receive the Check Request From and receipts to process the payment and mail the check.

Payment to an Individual: W9 Form Required
Please note if you are requesting a payment to a person for services (i.e coaching, theater costume design, teaching a seminar, etc.) the person must complete a W9 form and send it to the Foundation. The Foundation does not withhold taxes but does report payments to the IRS as required by law.

Sales Tax Exemption
Purchases made for educational programs such as the ones funded by the Foundation’s grants are exempt from Minnesota Sales Tax. When you are purchasing materials, let the vendor know the purchase is sales tax exempt and provide the Foundation’s ST3 form to show the vendor they do not have to collect sales tax.
